I think your edit improves the poem, the layout improves the readability. Some notes:

These seven lines are chockfull of childhood images/emotions.

I miss the unstained teeth a bit and especially miss the child speaking in poems but they are probably best left for another poem.
The past is running shirtless

in hand, asking the MC if he knows where
to find a grand piano, touching all the bridesmaids
and embarrassing itself.
The change from himself to itself is perfect for me and pulls the whole poem together. Happy to see the whole bump/baggie thing gone.

Not a fan of the accusatory tone, possibly from the previous version:
Time always leaves a little less than what you need,
why your desperate heart keeps coming back for more.
Desperate heart is cliched so maybe something else, but I like the compassion that "desperate" implies, I think that's where the poem lives now. Losing addict here deletes the last wisp of brother Mark but he lives to haunt another poem. I'm not sure I can read the new version correctly because I've already met him but this may be a better poem.
