I can;t tell if this poem is transphobic or not.
Properly interpreting the poem, people who use the term "transphobic" would call it that. In the real world, "transphobic" is a grossly misleading and effectively meaningless term of art... so "is" doesn't fit, but "would be called" is a yes.

Without going into the merits of the point of view, some thoughts:
1) the poem is one long, hard to follow sentence. Perhaps 'strikes one with its...' could be replaced by something like an em dash or colon after 'puberty' followed by 'mutilation is a lesser instance...' etc
2) I still don't quite get what 'the greater suicide' is. Surely the greatest suicide is suicide itself. Is losing gender identity a 'greater' suicide than losing a genital? Maybe it is, for the author, in which case reversing the articles i.e. 'the lesser instance of a greater suicide' might be more apt.
